Our social workers and geriatric specialists meet with clients at the clients' homes. After building trust and rapport, we help clients find other needed services such as help with house-cleaning, transportation, and linkage with doctors.
Community support clients often need help with organizing finances, arranging appointments, and coordinating several services; our social workers assist with these functions.
Clients often need advocacy with other providers; our staff often advocate for services and benefits on behalf of their clients.
Some of our community support clients also need staff help to obtain groceries, go to the bank, get to doctor appointments, etc.
Supportive Services takes place in client homes in the northwestern area of Franklin County.
